7 Rocky Mount, VA Rocky Mount, VA Full-time Full-time $36,720 - $56,473 a year $36,720 - $56,473 a year 9 days ago 9 days ago 9 days ago Geriatric Care Coordinator Position #FC-320-49 (Franklin County, VA) Position would serve as the primary responsible liaison between Piedmont Community Services and Catawba Hospital for discharge planning, coordination, and monitoring of services for Catawba Hospital patients and their families who reside in the Martinsville/Henry County, Franklin County and/or Patrick County areas.
Will assist in the development of collaborative relationships with area nursing homes, assisted living, and providers who can assist with guardianship appointment for patients needing this support.
This position would serve out of the Franklin County office and would also be responsible for providing restoration services to adults in the PCS catchment area.
Bachelor's degree in human services or a related field preferred with one-year experience of direct care, or any equivalent combination of training and experience which provides the required knowledge, skills and abilities.
Salary is based on education and experience as it relates to the position.
